Mask Exemption Forms

Dear Staff/Parent(s)/Guardian(s):

One of our top priorities is to be able to continue to offer in-person learning, five days a week for every student, while also continuing to be able to offer a robust extra-curricular program for students.   

In an effort to support this mission, the Schools Board voted to revise the mask policy at the September 13, 2021 School Board meeting.

The revised policy is:

Universal masking is required for all PK-12 students, staff and visitors in school buildings and on buses regardless of vaccination status. Exemptions for religious or health conditions require documentation.

Below is the timeline:

  • Forms to be submitted by 4 pm Monday, September 20, 2021 to the child’s building principal.
  • Forms can be found on the district website, and paper copies are available in each school office.
  • Approved accommodation notification by Monday, September 27, 2021.
  • If a form is not received and approved, students will be expected to wear a mask beginning Tuesday, September 28, 2021. This includes students who requested a medical or religious exemption at an earlier date.
